Observation

0165(2)(c)- Cords were hanging and accessible to children in both rooms. 0165(2)(d)-Plastic bags and diapers that were open that had plastic around them were located in the bottom drawer of the changing table area. The owner corrected this on site. 0165(3)(a)-Outside on the playground there were bolts sticking up on the bottom of each fence post that is bolted to the ground. The fence posts are along the outer part of the small playground area making these accessible to children. 0165(5)(a)-There were exposed electrical outlets without tamper resistent automatic shutters, blank plates or or tamper resistent cover that were accessible to children. 0165(5)(e)- There was a cord to the AC unit that was contected to an extension cord without a surge protector.
